College Transcript
- College transcript is required
- Transfers only. Scan all college or university records. Upload the scan as a PDF to the Wellspring portal. Transcripts should list all courses taken, grades received, and grading scale. Include any certificates received. Please provide English translations if necessary.
Financial Statement
- Complete the form and upload a PDF to the Wellspring portal.

If the student has been out of school for more than four months, please complete the Accounting of Time worksheet and submit it with your application. The educational history provided on the application indicates that the student will graduate/has graduated 4 or more months before the INTO program state date applied for OR there is a gap in the student’s academic history of 4 or more months. Please inform us if the student has taken additional educational courses or obtained any work history during this period. If so, please provide the dates of these activities. This information is required as part of the holistic review during the academic evaluation for admission Please complete the supplemental declaration form and upload to the student
